This is an online service is also available to people with diabetes that are registered with a GP practice in Scotland. It helps you to:

    • understand your medication better

    • understand how diabetes affects your lifestyle

    • learn how to manage your condition more effectively

    • see your own up to date diabetes results, letters and treatment plan (imported securely from your GP and hospital clinic records

To register you need your CHI number – a number unique to you which is used to identify you wherever you receive NHS care.

Ask the receptionist for a My Diabetes My Way letter. The letter will have your CHI number on it.  You can ask at the reception desk or telephone 0141 800 0840.

Go to www.mydiabetesmyway.scot.nhs.uk and click on the REGISTER button. Enter all your details and you will be given a unique username and password to allow you to use this excellent service.
