If you are unable to access either forms online you can attend our reception to collect paper form in person.

When you come in to the practice for the first time, if possible please bring an item of personal identification with a photograph on it, e.g. a passport, driving licence or bus pass.  Also, if possible, please bring an item with your current address on it, dated within the last 6 weeks.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Temporary Registration

If you are ill while staying away from home, you can register with the practice as a Temporary Resident. 

You can be registered as a Temporary Resident for up to but no longer than three months. This will allow you to be seen by clinicians in this practice while still remaining registered as a permanent patient with your local GP. After three months, if you are still living in the local area and want to remain on our practice list, you will have to permanently register.  

When you return to your own home, any medical records held by Keppoch Medical Practice will be forwarded via the Health Board to your own medical practice so that they have a record of your care here. 

To register as a temporary patient, please telephone reception on 0141 800 0840 or call in to the practice.

Please note that you cannot register as a temporary patient in the area in which you are already registered.  If you need urgent medical treatment however, we may see you on an 'immediate and necessary' basis.

Practice Boundaries

We are contracted to provide services mainly to patients who live in the G22 postcode areas. We will also accept patients living in postcodes G20 7S, G20 9, G4 9T & G4 9B due to their proximity to the health centre. 

The approximate outline of our contracted boundary is shown below by the yellow lines. The dotted line shows the G22 postcode area.   

If you live in, or move to, an address outwith the practice boundaries you will need to register with a GP practice that provides a service to that address. You will usually have 4 weeks to change your GP practice before you are  removed from the practice register.

If you plan to move out of Scotland please inform us so we can arrange for your records to be sent onwards or back for central storage.
