At present, you should only come into the Health Centre if you have been asked to attend. If you would like to register with the practice, please complete this registration form and the New Patient Questionnaire and e-mail them to the practice at ggc.gp43237clinical@nhs.scot

Please ask our receptionist for a registration form. She will help you to complete it if necessary. Or you can download the registration form and new patient questionnaire, fill them in and either bring them along with you or e-mail them to ggc.gp43237clinical@nhs.scot
When you come in to the practice for the first time, if possible please bring an item of personal identification with a photograph on it, e.g. a passport, driving licence or bus pass. Also, if possible, please bring an item with your current address on it, dated within the last 6 weeks.
Once you have completed a registration form, all patients aged 5 years or more will be given an appointment with the Practice Nurse or Health Care Assistant for a New Patient Health Check. The receptionist will give you a practice leaflet and a container for a urine specimen which you should bring to your appointment together with your medical card, if you have one.
Medical treatment is available from the date of registration. If you need to see a GP or Nurse before the date of your New Patient Health Check, please click on the Appointments button above for information on how to arrange this.
