Chronic Medication Service

The practice participates in this service which is offered to patients on repeat medications for long term conditions in partnership with the community pharmacies. 

If you sign up to this service, the practice will issue a serial prescription to the pharmacy which will allow them to dispense your repeat medication to you directly for 24 or 48 weeks, depending upon which period the GP considers suitable.  This will mean that you won't have to phone the practice to order your medication during this time.  If any prescribed items are discontinued during the lifetime of the serial prescription, this information will be sent to the pharmacy to prevent further dispensing of the items.

You may register with the community pharmacy of your choice.  You must have a confirmed long term condition (one which has lasted or is expected to last longer than 12 months) and be receiving regular medication to treat this condition, and you must be registered permanently with the practice.  The final decision on whether you are suitable to receive this service rests with your GP, however.

Your GP may suggest to you that you would benefit from registering with the service but you must go to the community pharmacy in order to register.
